All new clients are required to complete the Health History and consent form. Please check your email for the link.
Direct Billing Policies
Direct Billing requires a valid credit card on file.
A separate Insurance Authorization, Consent and Assignment of Benefits form must be completed to allow direct billing on your behalf. Please contact the therapist with the name of your Insurance to ensure the appropriate forms are emailed and completed BEFORE your appointment.
Saskatchewan Blue Cross has updated all Member ID’s and Plan information. To update your information, please contact the therapist to have the Insurance forms emailed.

New Clients & Reactivation
All NEW and DISCHARGED clients (those who have not scheduled in 12+ months) are required to contact me directly by phone or preferably by text to discuss treatment options and schedule appointments. (Please do not submit waitlist requests.)
Online intake forms must now be completed BEFORE your appointment can be confirmed.
New clients WILL NOT be accepted between July 1st and September 1st.
Manual Osteopathic Therapy
THESE TREATMENTS ARE NOT MASSAGE THERAPY AND CANNOT BE DIRECT BILLED!
Canada Life does not currently reimburse for these treatments. It is a clients responsibility to discuss coverage by CMMOTA members with their insurance provider.
These treatments ARE NOT MASSAGE THERAPY and do not use any oil or lotion and are performed fully clothed, therefore comfortable, non-restrictive clothing is required (NO JEANS).
Manual Osteopathic Therapy is a gentle, non-invasive, assessment based treatment focused on helping the body find balance to promote the body’s natural ability to self-heal. Treatments begin with an extensive intake/consultation and full body assessment to help determine the root cause of pain and dysfunction. Treatments are composed of a variety of techniques including:

These treatments are offered to current clients only.
Integrative Osteopathic Massage treatments are a holistic full body approach to healing! Following Osteopathic principles, gentle assessments and palpation are done to help the therapist determine areas of tension and dysfunction throughout different systems of the body. This allows the therapist to better understand and treat the root cause of the problem instead of the symptoms alone. Your treatment plan is determined based on the results of the assessment, as well as the consultation. This unique combination provides clients with a powerful healing approach that addresses pain and dysfunction in multiple ways. These treatments include assessment and treatment of the muscles, joints, connective tissues, nervous system, lymphatic system, and internal organs.
Canada Life does not reimburse for the Osteopath portion of these treatments.

If you are a NEW CLIENT or it has been 12+ months since your last appointment, you are REQUIRED to contact me to complete a health history and consent form before scheduling an Initial Appointment. Failure to do so will result in appointments being CANCELLED.
Traditional therapeutic massage therapy to help manage acute or chronic pain, injuries and muscle tension. These treatments DO NOT include any Manual Osteopath techniques. Manual Osteopathic Therapy must be scheduled separately.
