Booking Policies

Online booking is available to current/returning clients only. If it has been 12+ months since your last appointment, you’ve been discharged and your file is now closed. You are required to contact me to update your Health History and Consent form and schedule an appointment.

New clients are required to contact me directly to discuss treatment options, complete required Health History and Consent forms, and schedule appointments. Availability is limited.

Direct Billing now requires a valid credit card on file. A separate Insurance Authorization and Consent form must be completed to allow direct billing on your behalf. Please contact me to ensure you are sent the forms for your specific insurance provider.
