Please review the following important policies before booking your appointment.
📍 2025 Schedule is BOOKED!
- If you missed the reminder or couldn’t book in advance, please add your name to the waitlist through your Janeapp account.
- Clients will be contacted in the order requests are received — first come, first served.
🩺 Health History & Informed Consent
- All new clients, and those who have not booked in 12+ months, must complete their own health history and consent forms before receiving treatment.
- These forms help us provide safe and personalized care.
- Only the person receiving the treatment can fill out and sign these forms, not a spouse, partner, friend, or family member.
đź’ł Insurance Direct Billing
If you plan to use direct billing through your extended health benefits:
- You must request an Insurance Authorization & Consent form from me in advance. NO FORM = NO DIRECT BILLING
- This is required by your Insurance Provider and gives us permission to submit claims and share essential treatment info with your insurer.
- Coverage is not guaranteed — you are responsible for any unpaid balances, therefore a valid credit card is required on file.
- Direct billing is available for your primary insurance plan only- you are responsible for submitting to secondary insurers.
